All teams are required to complete the following forms in order to register with
the ASA through the Metro St. Louis Office. The forms are in Adobe Reader format (pdf). Forms
can be filled out either by hand after making a copy of a form on a printer or filled out the form on a computer and then printing
each completed form. Adobe Reader will not allow the filled in
form to be saved as a file on the computer. WARNING: All information on the form will be
removed if you close form before completing and and printing. After forms are completed and signed by the
manager they should be returned to the ASA Metro St. Louis Office. Contact the
